Teammate was using a tool to clear a clog that was causing plastic pellets to fall onto the floor at box dumper in compounding area. Once clear teammate stuck arm into butterfly valve auger section to check suction. LOTO violation.

After clearing the clog teammate stuck his right arm into the auger without disengaging energy and the butterfly valve closed down and trapped him at the wrist.
