Fabricating a metal rack to hold leg irons using metal and a wire welder.
What happened
The rack was positioned upsided down in order to attach the wheels. As the employee attempted to roll the rack upright from one end the lifting lug on the far end caused the rack to fall over to the side. The employee atttempted to get clear but was struck by the falling rack.
