What happened
n [REDACTED] the employee was responding to a jam-up on the incline conveyor that takes scrap insulation from the [REDACTED] stacking table to re-pack. The employee reached over the sidewall of the conveyor to remove jammed pieces of insulation by hand. When leaning her mid-section against the sidewall she felt discomfort in her rib cage and felt a “pop”. The employee did not report the incident immediately however she did report the incident [REDACTED] after the discomfort failed to fade. Once the employee reported the incident she was immediately seen by the onsite occupational nurse. Due the employee’s particular medical assessment and history the employee was taken to urgent care. X-rays of her mid-section were negative though she received Rx muscle relaxers. The employee returned to work on normal duty
