The forklift was doubling a stack of 20 ft trusses and had the fork extensions attached The employee had to add a block under the stack so the driver could back the fork lift out The employee went under part of the extension to add the block and stood up and backed out and struck the back of his head on the fork lift This caused him to become dizzy and he started to vomit
