EE was operating Header machine #641.

EE was changing the Trim Die for Machine 641. EE stopped the machine EE opened guarding to the Die Pocket at the Operator Platform. EE turned off power to the machine but forgot to turn off air pressure to the trim die. EE started to unscrew the two (2) locking bolts holding the Trim Die in-place. Because the Operator did not turn off the air pressure per the machine instructions there was potential energy stored within the Trim Die so that when the lock bolts were removed the potential energy was released and the Trim Die (#4) hit the operators left hand causing an impengement injury to the middle and ring finger on the left hand.
