EE was assisting another employee removing the form tool from Former 10 by installing the removal wheels to the tooling. EE right elbow was resting in the opening between the form tool and upper platen. Neither the EE or employee they were assisting had put the stop block pin in place before entering the former.

The employee the EE was assisting unintentionally pressed a release valve button that caused the upper form platen to close slightly. This caused the EE 's right elbow to be struck by the platen and caught between the tooling and platen.

Fracture to the right elbow.
