[REDACTED] was working under the mono beam and had the grating lifting up resting on the angle iron to give him more space to work. Another maintenance employee was not aware of the grating resting on the 1 ' ' angle iron and stepped on it causing it to slide off hitting [REDACTED] in the top of the head.
