Before the incident
A forklift operator was in the process of exchanging out an empty propane for a full one in the yard propane rack area.
What happened
While pulling the full propane tank out from top of the rack the 55lbs propane tank slipped out of the TM’s hand and dropped striking landing directly on the TM’s left toes.
