Working on rolling scaffold
What happened
There were two employees who were attempting to use a piece of rolling scaffolding to work on a tank. Before beginning work the employees adjusted the platform height. It appears that one of the employees did not properly insert the locking pin that supports the platform on the legs of the scaffold. Therefore shortly after the employees mounted the platform the platform slid down causing the employees to fall. The injured employee hit his left arm elbow on the tank causing a contusion and sprained ligament.
