What happened
Employee was attempting to transport a stack of 1 4 thick 24 diameter manway lids from Bldg 12 to Bldg 6. He placed them on a hand cart on their edge to transport them. When he was placing the cart down the lids began to tip forward. The employee attempted to prevent them from falling but he was not successful. The manways contacted his right foot causing a crush type injury.
