What happened
The employee was using a pipe wrench while attempting to loosen the nut in an awkward position. As he applied force to loosen the bolt the tool lost the ability to grip because the tool was turned in the wrong direction causing him to lose his footing. The work area was covered in hydraulic fluid which was in the process of being repaired. The employee fell about 3 feet into a containment pit suffering an injury in the head.
