Before the incident
[REDACTED] had been sitting in [REDACTED] staring at the wall when this RN arrived just prior to [REDACTED]. She had finished dinner tray this RN introduced self and cleared tray - pt said thank you. [REDACTED] continued to stare at wall almost appearing in a catatonic state.
