Before the incident

The EE was headed out to the plugger debris box at the end of the plug line outside down the breezeway near the millwright shop.

What happened

EE was going outside to push the excess plug waste down in the plugger debris box. Just prior to this a water line broke because it had frozen over night next to the plug debris box. The sun thawed the pipe and water flooded the roadway in the breezeway between the pluggers and room #2. There was a sheet of ice that was covered by the water. The EE walked through the puddle not realizing that there was a sheet of ice on the ground under the water. He slipped and fell on his back. Another plugger came over and helped him up. He then called his supervisor. We checked on him and made sure he was okay. We then got danger tape and cordoned off that whole area of the breezeway. We then instructed the pluggers to call a forklift driver to shake the box down with a hyster going forward. He was okay but he was soaked from being in the puddle so he was sent home.
