Before the incident
TM was removing material lines when a clamp slid down a vertical line and caught his fingertip ripping his nail. TM was holding moving the lower part of a vertical section of material piping. Maintenance was at the top removing the line connection so the section could be removed. There is a rubber liner inside the clamp that released from the clamp allowing it to fall down the material line. TM struck by the falling object.
