What happened
[REDACTED] was in the room with an RN transferring a patient from the chair to the bed. They utilized the MATS system and RN was pulling on the side of the bed as [REDACTED] was pushing the patient from the chair. After a time out they double checked patient 's lines and tubes they inflated the the MAT and initiated the transfer. While RN was pulling the patient she noticed resistance and it felt like patient was stuck on something preventing him from passing over the chair into the bed. [REDACTED] then moved to the RN side where I was pulling and attempted to pull once more which caused the patient to slide down the MAT. They shouted out for help to prevent the patient from falling in the middle of the chair and bed. At that point RN had moved by the patient 's legs in an attempt to prevent the fall. HS and another [REDACTED] came in to help and were on the patient 's right side and other [REDACTED] was still on the patient 's left pulling the patient. [REDACTED] attempted to pull the
