Before the incident
The bologna pockets on [REDACTED] were not forming correctly causing the meat to not fit into the pockets. While troubleshooting the issue the guard to the side of the forming station was removed by her supervisor for visual inspection to ensure the correct forming plates were in place. The correct forming plates were originally installed but needed to be removed to replace with a smaller plate to see if that would correct the issue. While the operator was changing the plates out the supervisor was checking the recipes on the panel on the opposite side of the operator to ensure all settings were correct. Once the settings were checked the supervisor misunderstood the operator by thinking she was ready to start the machine. Once the machine started the operator verbally communicated to the supervisor that her arm had been in the machine when it started and the supervisor immediately hit the E-stop button after the operator was demonstrating signs of severe pain in her right arm.
