Before the incident
Extruder operator was assisting on [REDACTED] At approximately [REDACTED] the employee had just completed a cut over and started a new roll The operator had sent the other operator to break and noticed that the roll was starting to telescope The operator had then stopped the winder and cut the sheet to see if he could straighten the sheet and prevent it from further telescoping After cutting the sheet the pull rolls were not able to keep the same speed of the chrome rolls resulting in the sheet losing tension in the haul off This resulted in the sheet wrapping the bottom chrome roll and pulling the sheet backwards The operator had seen that the sheet was starting to go backwards towards the chrome rolls and he was attempting to pull the sheet in the opposite direction to save the line the operators hands got to close to the pull rolls and then pulled his left hand into the pull rolls Employee reacted and was able to pull his hand out before going in further this resulted in laceration
