Before the incident
Machine alarmed for a NG part pressure sensor on the end of arm fixture. The robot stopped moving when the alarm occurred and the part was positioned above the molding die [REDACTED] associate confirmed the machine had stopped reviewed the alarm code and the part was positioned above the molding die. that the associate removed the actual part from the fixture and then re-entered to blow out the fixture with an air hose.
What happened
With the fixture being 6~8” higher that the part it required the associate to stretch farther into the machine. At the angle the associate was positioned it cause outward pressure to the platform and it slipped out from under the associate. This caused the associates face to hit one of the guide rods on the mold causing the laceration.

Laceration above the right eye – required 4 stitches
