What happened
Hand was pulled into nip rollers that press glass together on Interceptor line that squeezes two pieces of glass together on the spacer. There was butyl on a roller and after he cleaned the rollers he left guards off to observe the rollers running and while looking over the top of the machine his left hand was holding a rag and it was pulled into the rollers while machine was running.
