EE was offloading bundle for machine 3401 When lowering the bundle the chain from the hook got stuck between the bundle and the lumber EE lifted the bundle back up and went to strike the chain that was caught between the lumber and the panel with a hammer At that point the hook wasnt secured properly and the bundle slipped and smashed his left middle finger
