Before the incident
Went into the patient room to put on the mitten restraint untied. Then asked Nursing Assistant to assist with the mittens since the patient needed repositioning Nursing assistant adjusted height of bed & then repositioned patient.
What happened
Pulling up patient in room 2520 with NA. Felt something pull on the left lower back. Immediate pain difficulty walking unable to bend and sit due to the pain.
