Before the incident
Worker was holding a pipe wrench with both hands as another worker was attempting to remove a 4 inch stainless steel nipple out of a 4x2 bell reducer that was secured in a vise on a work bench.
What happened
As the other worker was pulling with a separate pipe wrench the adapter secured by the vise slipped which caused the affected worker 's fingers to wedge between their pipe wrench and the edge of the metal work bench.
