EE was assisting w cleaning a patient. She stated to feeing nauseous hot and dizzy. went to go sit down for a moment in the patient room and then she just remembers waking up on the floor with a headache. EE is claiming head and neck inj from fall.

EE called supervisor at [REDACTED] and stated that she had gotten dizzy and fallen on the floor. Upon arrival to the room (115) [REDACTED] was lying on the floor (by the window). She stated that she hit her head and had neck pain. Employee B P and blood sugar was check by HS on shift right after injury and everything checked out fine. [REDACTED] called in to transport
