What happened
On [REDACTED] at approximately [REDACTED] [REDACTED] was cutting wood pieces. The equipment that the employee was using was the rip saw. The employee was working with another employee. After they finished [REDACTED] began performing cleaning duties. The employee deactivated the power. The employee detached the vacuum hose and was using that to remove saw dust. The employee was holding the vacuum with his right hand and using his left hand to scoop the dust. At this time the employee reached far under the top portion of saw. He struck his fingertips on the blade. This resulted in the amputation of his left middle fingertip and his left ring fingertip.
