Installing a bolt underneath the chassis
What happened
The employee was laying on their back while attempting to install a bolt in the heat exchanger. The employee was using a pry bar to move the hosing out of the way in order to access a hole for a bolt on the underside of the exchanger. While doing this the employee pulled on the prybar causing it to slip from its position and strike the employee in the face.
