The injured employee was responding a call from a line operator having issues with his machine.

The injured worker was troubleshooting damages on line 436. The area of the machine that was having issues was the beller gasket loader. The beller is the area of the machine where one end of the pipe is expanded so that the pipe can be installed end-to-end. This type of pipe required a gasket ring be seated into the belled end of the pipe. The injured worker was inspecting the rubber gasket by reaching through the opening in the gasket ring holder. While feeling around the rubber gasket for leaks the gasket ring loader mandrel closed on the employee 's left foreman. The employee 's left forearm was caught in between the gasket loader mandrel and the gasket ring holder.

The employee 's left forearm was caught in between the mandrel and gasket holder on line 436 at the beller. Breaking the Ulna and Radius. Including laceration to the area on the forearm that was crushed.
