Employee was working with a tooling die. He is required to work on it and transfer it to the press. This die was one of the heaviest when the incident occurred. Die must be moved from the workbench to the die cart.

Employee was pushing the trunnion die onto the die cart. The die cart moved backwards and the die was only half on the cart and was about to fall off so the employee was straining to keep the die from falling on the floor. The employee had assistance; however the assisting employees side did not move causing the die to not make it all the way onto the cart.
