PT was sitting in recliner staff transferring Pt from recliner to bed with hoyer lift hoyer would not life Pt all the way off the recliner so staff was attempting to move the Pt the Pt was slipping out of the bottom of the sling employee stood on the bed pulling the Pt toward the bed while 3 staff members were pushing the PT towards the bed.
