Before the incident
The site began an autoretractable knife trial on the morning of [REDACTED] The knives were selected in accordance with SW Standards and provided at no charge by [REDACTED] for trial purposes The knives were tested upon arrival and determined to by fully autoretracting and functioning properly The trial was requested by a Unit Manager for the purposes of breaking down thick cardboard boxes where employees struggled to cut the material with concealed blades On the night of [REDACTED] an employee at the robot was given the knife to test The employee placed a pallet of gallon cans on her forks at chest height and proceeded to cut the outer stretch wrap material with the new knife The employees downward arm motion and the blades failure to retract led to a laceration above her right knee that resulted in eight sutures The employee returned to work with no restrictions approximately two hours after the incident occurred
