Before the incident
Employee was using a pneumatic chisl to clean the shaft of a sand mixer.
What happened
After cleaning the sand mixer shaft and confirming that the sand was mixing properly an oversight led the serious injury. While checking the condition of the sand chute he mistakenly believed that he turned off the mixer. Unfortunately as he reached into the chute his fingers came into contact with the mixing shaft paddles resulting in severe lacerations.
