What happened

Patient up to nurse 's station requesting to speak with a specific nurse. Since this writer is her primary nurse this writer went out to the corridor 2 lounge to offer my assistance. Patient was sitting on a chair in the corner of the corridor 2 lounge by the television and stared intensely at this writer. When asked again what she needed assistance with patient quickly lunged out of her chair toward this writer tipped both tables over toward this writer and began to hit and scratch a. Patient continued only until this writer was able to grab a hold of her shirt which got pulled up and over her head before completely being taken off. Patient then stopped and went to her room. Provider was notified house nursing supervisor was notified of the incident and security was notified. My injuries: Scratches to forehead nose upper lip left side of mouth and chest area were scratched up and bleeding in all areas.
