[REDACTED] was beginning a welding project on CAT 30 that had a puncture in the fuel tank on the bottom outside corner Hot works permit was not filled out nor any protocols followed [REDACTED] believed that 6 feet clearance was sufficient but the permit clearly states 35 feet He was doing a tack weld and had looked at the area went back to welding and looked again the debris was on fire He grabbed a fire extinguisher that was near removed his welding gloves to pull the pin and weld
