Driving

Driver got stuck in the mud during a delivery. A shimmy was noticed while driving back. The driver was instructed to have the truck cleaned as mud was a possible root cause for the shimmy. The driver did not get the truck serviced and continued to drive with the shimmy for approximately a week. The driver called off due to back pain from driving the truck with the shimmy. The driver was instructed to get a doctor 's note for the call off. The driver received prescription medications and a restriction due to the back pain.
