Working in the Plastic Guard Department cutting stock plastic tubes to length to fill customer orders.
What happened
Around [REDACTED] on [REDACTED] an employee was using a plastic guard cut off saw 1163 cutting 3.25 x 106 long plastic tubing into 10.25 lengths. When cutting the 10 pieces the employee was using the right foot to engage the saw blade with a foot pedal. After the 10th piece was cut the employee took their left hand and reached inside the sleeve fixture that is 8.5 in length to remove a drop-off piece that was inside the sleeve. While the hand was inside the sleeve the employee accidentally stepped on the pneumatic saw pedal engaging the upward-moving saw blade which cut off the middle and ring finger and partial pinky finger.
