What happened
A dock driver was manually lowering a dock plate with the help of another dock driver at [REDACTED] (Main Building). While the affected employee was holding the dock plate the forklift driver attempted to use the forklift 's forks to assist in pushing the dock down. In doing so the forklift driver inadvertently drove the forklift onto the dock plate causing it to fall onto the affected employee 's left foot.
