The employee was lifting a box and reported feeling lower back pain.

On [REDACTED] at approximately [REDACTED] a supervisor notified me that the EE was having lower back pain. I asked the supervisor how it happened and they said that the EE had injured his back in the past and that it might be related to that injury. The supervisor later added that the EE wanted to go home early and take the rest of the week off. I said it was okay to leave. I clearly told both of them that if it was truly a work-related injury they would need to let know so I can file a report. The employee was going to let me know but never made contact. The employee went home [REDACTED] and submitted his request for time off which I approved the next day. I believe that if the employee would have reported this to me first thing in the morning I could have made my report at the time of the incident. Also after talking with the two employees together it was unclear if this was a work-related injury or an injury from the past.
