Before the incident

Standing on the forks of a forklift to reach the strap of a truck roll up door.

What happened

Employee was attempting to close the rollup door on our dock. They stepped up onto forks of their forklift to get some leverage when the trailer suddenly pulled away from the dock causing them to fall out of the facility and lad hard on their left foot which led to a fracture.
