Operator strained their left calf on [REDACTED] from packing out a 2.75lb part then stated they wanted to go to Ocupational Health for this on [REDACTED] where employee was released with no restrictions.. Then on [REDACTED] employee stated they reinjured their calf and went home for the day and requested to go back to Occupational health on [REDACTED] which is when Employee got put on restrictions we could not meet which started his loss time Had a recheck on [REDACTED] and was given same restrictions then again on [REDACTED] still with same restrictions. Restrictions where lighted to where we could meet these restrictions on [REDACTED] - Employee then refused to come back to work even though we where able to meet his restrictions stating he would rather sit at home and heal rather than come back to work so at this time on [REDACTED] the lost time stopped due to us being able to meet restrictions but employee refused to come back.

Employee stated they were grabbing more parts than they should have and was not walking around the bin to pack out but instead they were streching from one side of the container to the other adding extra stress on their body.
