An Injection Operator swapped out an empty gaylord with a full gaylord at the tippers. While preparing the empty gaylord with a new bag the operator noticed leftover preforms that were still in the bag. She removed the bag from the gaylord and proceeded to dump the leftover preforms into the full gaylord that had already been placed in the tipper cage area. Once a gaylord is moved into the cage area (light curtain broken) there is a 15 second delay then the tipper head automatically comes down on top of the gaylord. This is an automatic function that prevents operators form forgetting to hit the start button on the tipper.

The team member reached into the cage area to dump the left over preforms during that delay and the tipper head came down trapping her arms.

Team member was injured by the lid of the tipper coming down over her arms causing a tear in her left elbow
