What happened
Employee was backing into a plate cart using the tugger (A) in order to move the cart for the IRCO offloader. This task is performed multiple times daily. The cart stuffer backs the tugger up to cart and uses a no touch tool (B) to place the plate cart tongue (C) into the tugger jaw (D). When she was turned to perform this task she believes that her left hand engaged the throttle. The tugger jerked and her right pointer finger pinched between the back of the tugger and the no touch tool (F). Employee was wearing gloves and the gloves were intact with no visible punctures or tears (E).
