Before the incident
Employee was operating a fork truck to unload trailers. A trailer backed into dock and employee went to removed door seal from trailer.

Employee couldn 't find seal cutter so he used a document holder in an attempted to snap the seal by twisting it. Document holder popped open and the ends of the holder cut each of the employee 's hands.
