THE EMPLOYEE STATED HE AND HIS CO-WORKER WERE ATTEMPTING TO TAKE APART PALLETS WITH LARGE EMPTY BOXES ON THEM. WHEN THEY GOT ONE SIDE OF THE BOX LOOSE FROM THE PALLET IT HIT THE EMPLOYEE IN THE MOUTH AND HE FELL BACKWARD ONTO THE WAREHOUSE FLOOR. HE ALSO HIT HIS LOWER BACK ON ANOTHER PALLET WHEN HE FELL.
