THE EMPLOYEE WAS PULLING OUT A PALLET WITH THE PALLET JACK. THE PALLET JACK STOPPED SUDDENLY AND WHEN SHE PULLED SHE JERKED HER BACK. WHEN SHE WENT TO LIFT A BOX AFTER SHE FELT THE PAIN IN HER BACK INCREASE. SHE NOTES AS WELL THAT SHE HAD CHEST PAINS 30 MINUTES AFTER THE INCIDENT.
