What happened
There was a floor mat (anti-slip mat) on the ground at the checkout register in Coffee Department (Cafe Area). As stated by the employee when she came in for her shift initially she had not noticed the floor mat. The employee stated that as she was walking towards the register her left foot was on the floor mat and right foot got caught under the mat. She lost her balance and fell on her left side. The employee stated that when she realized she was falling down she ensured not to hit her head. The employee fell on the ground on her left and hurt her left elbow left shoulder and neck.
