Attempting to break a plunger adapter loose in the maintenance shop
What happened
While breaking an adapter loose on a Tank 1 plunger mechanism with a 3 4 inch breaker bar and extension handle the adapter broke loose from the part while applying pressure causing the employee to slip to the ground. When the EE slipped to the ground on to his knees the breaker bar handle made contact with the EE forehead causing a laceration.
