What happened
Operator was moving containers from the BU row to the [REDACTED] row. He had container [REDACTED] locked on and trying to put the container in [REDACTED] in the stack. The operator claimed he was going slow and that he applied the brakes gently. Also he claimed the container may have been too heavy. After applying the brakes his top loader which was H-01-128 fell forward leaving the back tires in the air. During the fall the bar damaged containers TCKU128888-3 MEDU844749-3. Furthermore HLBU187124-3 TCNU610222-1 [REDACTED] were also damaged during the incident. The top loader also appeared to be leaking hydraulic fluid. The operator stated that he was free of injury for about 20 minutes. Thereafter he stated that he was having neck issues and a headache. He was evaluated at the Wellness Center and was instructed to go home and be reevaluated on [REDACTED]. The claim number for this incident is 23-586.
