Employee had gone to the doctor on [REDACTED] and had told me her arm was hurting when returned she told me they had given her a wrist band and she had to go in for further follow-up to diagnosis what was going on with her arm no mention it was work related. 7 31 we spoke again and she told me that she went to the doctors on [REDACTED] and she was diagnosed with carpal tunnel at that point I asked her if she felt it was work related and she said yes. it has been bothering for some time. I told her we need to report and as a worker 's comp claim. Physician recommend wearing a splint and needs an ergonomic evaluation for her work area.
