What happened
The employee was on top of the loader 's platform during this time walking backwards - side-wards from the side of the loader(where the hydraulic tank is located) to the cab. As the employee was walking they missed the top of the loader 's platform and instead of stepping on the platform stepped on the ladder steps which threw the employee off balance causing the employee to fall from the top of the loader.
