Before the incident
Employee along with his team members were standing along side of a walkie jack that had been lift in the air by the forks of a forklift. The equipment was lifted to assess repairs needed.
What happened
The walkie jack became un-balanced while on the forks and fell to the floor. Employee while moving out of the way feel to the concrete floor.
