Working on knockdown of an intersection

[REDACTED] was working at intersection [REDACTED] for a knockdown. He went up in the bucket to cut the wires and as he was putting his (pork chop) tool on the messenger wire a Semi truck tried to make a left turn on [REDACTED] from [REDACTED] The bucket started moving back and forth. After the truck made the turn he placed his Jib Line on the Pork chop. After he cut the messenger wire the jib swung back to the left and him him in the mouth causing his front tooth to be chipped. He fell into the bucket after impact. He inspected the wires tools and realized the pin he put in place was gone which caused the jib to swing back.
